Well, I wish I had more to say about it. We've still never seen two original MKI's that are exactly alike, but this one is very very similar to the 'Groundhogs' pedal that Dave used to own, pictured much earlier in this thread.
I need to do some follow-up detective work to unpick the provenance behind it. The seller owned it for decades, and used it during the 1980s, but recalled its original owner being a guitarist who was known only as 'Bonk', but who played with a group called the 'Diddley Daddies' (presumably Bo Diddley-inspired).

cool story so far - fwiw Diddley Daddy was the follow-up 45 to Hey Bo Diddley in 1955 - the Stones covered it in 1963 and their version is on lots of bootlegs - i used to have on tape in the 80s...
